About
Our Story
The idea for Connie didn’t come from a venture-backed startup or a Silicon Valley lab. It came from the leaders of Community Based Organizations (CBOs), working in aging and social services, who were seeing firsthand where systems were breaking down.
And they understood what was at stake.
Not just efficiency, but access.
Not just funding, but dignity.
Not just data, but the survival of community-based care itself.
Today’s Reality
CBOs have always been and will always be the trusted front door for people navigating social services. They do the work healthcare systems can’t reach and often aren’t designed to handle.
But demand is rising. Expectations are changing. Digital systems are becoming the default way care is coordinated, funded, and measured.
And most CBOs are not built for this reality. They’re not designed to operate like call centers, and perpetual technology investments are impossible to fund and staff.
The strain on community-based organizations isn’t theoretical. It shows up as unanswered calls, burnt out staff, and clients turning to emergency services.
Across the country, CBOs are facing:
A rapidly aging population
Explosive growth in need for community-based services
Consolidated services that require
a no wrong door approach
Healthcare systems and state agencies that require digital coordination
What Makes Connie Unique
Connie does NOT:
Require large IT teams or extensive training
Force rigid workflows
Price organizations out of participation
Prioritize speed over care
Instead, Connie helps CBOs translate their work into the digital language healthcare systems and funders now require — without sacrificing mission, relationships, or values.
Challenges We Address
Care within CBOs is often provided in siloed departments.
Healthcare systems, payors, and government agencies are rapidly building digital, closed-loop ecosystems that require you to break down these silos.
Connie lets you communicate in real time, share data, and demonstrate outcomes.
CBOs that rely on phone trees, shared inboxes, spreadsheets, and manual tracking are increasingly seeing a breakdown in their systems… and there is a real cost associated with that.
The cost to using outdated communications systems are real:
Fragmented tools and inconsistent workflows are inefficient and multiply time needed for administrative tasks
Clients do not receive the full level of care that they require, putting them into a revolving door of need
A connected and accountable approach to social care is no longer optional. It’s essential.
CBOs are not able to access the full level of funding available to them
CBO staff are facing rising levels of burnout and leaving the sector, causing critical workforce shortages
Backed by Leaders in Aging & Community Care
Connie was created through a partnership between Nevada Senior Services (NSS) and the American Society on Aging (ASA), with key contributions from a variety of other respected organizations.
Together, they brought decades of operational experience, policy insight, and sector leadership to the design of the platform. This foundation ensures that Connie reflects the realities of your organization and they systems you already have in place.
